This service is for those diagnosed with or with a family history of adult onset disease.
1. Pre-counseling Question: We would to hear your key questions and concerns as soon as possible so please submit these.
2. Collection Clinical Information: Please provide any testing that has been completed
3. Genetic Counseling Appointment: We will completely review your pregnancy and family history and address your questions and concern. We will discuss available testing options and help you identify your best choices.
4. Filled Requisition and Clinician Letter: Navigating the testing process is smoother with meticulously completed requisitions and clinician letters, ensuring a seamless journey.
5. Letter of Medical Necessity (If Needed): If insurance coverage is a consideration, we will provide a personalized letter of medical necessity where indicated.
6. Post-Test/Results: We will call you with routine results and discuss any next steps if needed.
